Nia on her fav play mat...

When my mrs told me that we need to buy play mat for nia,
I was wondering... What benefit could it bring
What is the difference of playing on the normal mat compared to the playing mat ?
Why do we need to spend few hundred buck to buy something unnecessary ?

But then... Everytime those question come into my mind...
I always remember that ... My mrs , unlike me... She reads a lot about babies... She spends her pregnancy months doing research about raising a child.
Definitely she knows better.
Not that I m a bad father
Man does what man does rite?

So yes., when my wife said we need a play mat... I bought one for nia..
For nia... Price comes second. Satisfaction and quality is number one ! Not that I am bloody rich, I just want the best for nia.

And guess what... Nia loves her play mat
She becomes very excited when we put her on that thing
Yes... Those colorful stuffs makes her happy.
When nia is happy... Mommy n daddy are the happiest people

Sometimes, we ( the husbands) need to listen to our counter part...
Most of the time.... They better listen to us!

Lazy Saturday

Gosh, I love superbike
I nearly bought one recently
But my mom was against it
Then I ended up getting miss blue

Apparently my mom was right
Miss blue was a better choice
Considering my life has changed
It used to be only 2 of us
Now it's already 3 of us
Need ra larger transportation

Nia's stuffs took half of the miss blue capacity when we travel
The stroller... The bed....
Where do I put nia should I bought a Ducati instead ?

Yet and still
Maybe in the future
Somewhere nearby future
I might get one superbike
Providing there is a cheap good deal
Being typical Malaysian
We want something good, at the lowest price
We ll see about that

For the time being
Superbike is not a must yet
It's not a necessity
Just a luxury item
But it would be nice to own one
Riding it everyday to work
Cool isn't it?

Why so "takut" one?

It comes out in the paper today, the MPs need to discuss between themselves regarding either to declare their wealth or not prior to the next coming election.

The SPRM has come out with a suggestion that all MP must declare their wealth to avoid future corruption.

Of course , as we all know
MPs are human too
And human is greedy
Greed makes people do stupid things
Such as accepting bribe
Not saying that our MPs are taking bribe
But if they don't... Why is it do hard for them to declare their wealth .

One of the reason being stated by them is due to security reason...
Owh come on... Our 1 Malaysia citizen is not that stupid rite....

Nia is growing up so fast

I do feel a bit sad and dissapointed
As I didn't spend much time with nia and mommy
Daddy is too busy working

It does reflect my situation right now

I came across this article in the newspaper this morning
A husband threatened to slaughter his wife due to jealousy

Bloody hell
For crying out loud
If u hate ur wife that much...
File a divorce and find a better one

But then
In this case the husband is a drug addict

Our government should make a new law for this drug abuse people
They should be sentence to jail instead of pusat pemulihan
Better still , send them to the jungle
Start a new life as a Tarzan

I am against drug abuse
I hate those people who is a drug addict
When u r on drugs
U do crazy thing...
Seriously the gov should do something about these people

These drug addict , they are the propel who do all the crime.... Robbery, rape...

The best would be death sentence for those drug addict

It has been raining non stop since yesterday

Was a quite cold cloudy morning
Doesn't really match my mood
I am pretty excited to start working at this new place

Do not enter! ICU is not a playground

Yesterday was my last day being incharged of the ICU
Damn I am gonna miss it
No ICU at hospital Tanah Merah
Only Ot.... And Ed
Less work for us , Anaest

One thing that irritate me when I am incharge of the ICU
Seeing pregnant ladies and children visiting patients in ICU is really scary
The risk of infection when u visit ICU is very high
The patients in the ICU commonly infected with premier league bacteria such as acinobacter and mRCONs
This bacteria is really dangerous

Worse case scenario of u are pregnant
U r risking urselves and also ur baby when u r visiting patients in ICU
Pregnant ladies and children are at higher risk of getting infected
And when they do got infected, the illness is very severe

I am not saying that u can't visit ur relative who is dying in the ICU
Not saying that the children has no right to see their severely sick parent ....
But we have to consider the risk of going inside the ICU
Is it worth it to be infected ?
Bare in mind that visitors in ICU usually do not follow the hand washing rules

So, a take home message for the day... If u r pregnant and have small kids, stay away from ICU

My last day at Hrpz ll

Of course I am excited to go to hospital Tanah Merah
But I do feel a bit sad
Having to leave Hrpz ll
A place where I actually started my life as a doctor
A place where I learned a lot of thing
A place I never forget

Hopefully one day I ll get the chance to come back here again
Should I get into the master programme
I ll be back here as soon as next year

But then
I haven't make up my mind regarding furthering my study

Just go with the flow
For the time being

I ll just enjoy the moment
Continue treating people

Finally , eventually
I got my transfer
I handed in my transfer request last season ( mcm Wayne Rooney la plak)
And I finally got it

It has been a lovely 4 years in Hrpz ll.. From 2008 until 2012.
So many memories here
Good ones, bad ones
Being screwed, got screwed
And screwing other when I am a senior
Just a cycle of life as a doctor.

It is not that I hate Hrpz ll that I requested to transfer
Damn I love this place
I love my Anaest department
The best department in the world
No where in the world I would find such a family like department.
Going to work everyday seems like " balik kampung"

We have breakfast together... Helps each other in ICU and Ot ..
Such a harmony condition here.
It is a comfort zone for a lot of people here.

But then...
We have to let go
In order to gain new experience in life
I ll be going to hospital Tanah Merah starting next week
Will be the new Anaest there
So excited to work at a new place

We all love to visit sick people at tr hospital
It is such a good thing
When one of ur family member is sick
The whole village come and visit
Something that we have been practicing for so many generation .

Nonetheless ,
While u are waiting for ur family member at the hospital
There are many benificial stuffs u could do at the hospital

Let's say if u are waiting for ur mom who is admitted to ward...
During morning time.. While doctors are doing their round
Family members are supposedly to empty the ward
During that period
Why don't u pay a visit at the blood bank
And donate ur blood
It s such a meaningful thing to do

It is not far way from the ward
It is not difficult thing to do
It is not dangerous for sure

If u are just sitting in front of the ward
U r not helping much by that
If u donate ur blood ..
U might have save a life, or two...

I came across this article in utusan Malaysia few days ago.
Pretty much concerning about ladies who spent a lot on cosmetic to look prettier

Honestly, I have nothing against it unlike most men who disagree when women spend a lot on cosmetic .

We all love being pretty
And being men... It is our nature , we love to see pretty ladies. Nothing new there

Hence, If ladies want to spend thousands per month on cosmetic , let them be... The deserved it. They work hard for it.. Let them have fun with it.

If men want their car to look prettier than others,
Then women wants plenty of make up as well

Everything is fine as long u r not being thrifty and spend beyond ur limit

It has been thrilling... Tiring... But full of joy and happiness
Seeing my little princess growing up everyday.
Of course, comparing what mommy did and what daddy has done to nia so far.... Daddy seems to do very little

It got me to realize what Allah said "syurga terletak di bawah tapak kaki ibu"
U will only understand this once u ate a parent
It is not easy to raise a baby
Nevertheless , in the end of the day... The cuteness of the baby took away all the pain

Good intention and good riddance , those four words do not always come together

There are moments in life... When u do something with purely good intention... But in end of the day... U end up in the courtroom. Worse case scenario... In jail.

That is life . It sucks but it is true

For example
Life as a doctor .
People expect u to save life
And we do.. All the time
No question about that
But then,
Nobody remember how many life we have saved
Nobody keep track on that
But once u screwed up..
It will be on ur record forever and
People will remember u as a doctor who screwed up.

I knew a few doctors who refuse to take or treat critical cases
Just to keep their record clean and clear
U can't blame him
We doctors have family as well to take care of
We need to keep our job safe

Plus, in this new SPBA system, we want high marks for our skt
Hence we will get higher salary every year

It is a normal human instinct
We want to be safe before others

No one to be blame for this

The system has made our life so rigid
Everything must be done accordingly

Rules comes first... Other people life comes second

Cruel...
But we all need to keep our periuk nasi right
How I wish all doctors could work
As freely as u seen on tv... House md for example...

When "good" is not enough

Just signed the SPBA
Much has been discussed regarding the SPBA.
Some said... It brings more benefit to the government and less for the citizen like me.
Some said... It is a way to make sure that the government servants work harder
Some said.... If u have to choose between two evil.. Stick with the one u knew... The old school SSM

I signed for the SPBA
Why?
Because everyone does!
Well that would be the common answer!
Basically in the SPBA ( who those who r not working in the gov sector) it is a new system where they government want to reward the good workers and easily get rid of the bad ones

The higher ur skt marks... The higher ur salary raised..
If ur skt marks is only satisfactory... U will be given offer to quit ur job! Or the gov can fire u....

In a way.. It s a good way to improve productivity
But , does it...

Skt marks depend on ur boss. Some bosses are very good and they give skt marks to their worker.. With honestly and no bias...
Unfortunately.. Bosses are human as well.
In this system.. The bosses could easily give u bad marks and u will br out of job or u won't get ur salary increment, should ur boss hate u or u r not in a good term with ur boss...

Well.. That's the dilemma.
Fortunately my boss is a very good boss! Heheheheh
This is so true... Tiada niat untuk puji utk Dapat markah skt lebih occay..
My boss is a wise man.. He is trustworthy and I believe he gives people what they deserved.
Which is the reason why I choose to stick with SPBA and remain working at the gov hospital

HIV via tattoo???

is it possible to be infected with HIV via the tattoo's needle ?
well , that what happened to an australian tourist who went to Bali for a holiday , got a tattoo there
came back home and later on found out she/he got HIV
at least , that what he/she claimed

nonetheless...
clinically , is it possible?

let's go to the basic here. how does HIV transmitted? occay , easy as abc ... number one.. blood to blood... mother to baby and the famous unprotected sex
then,
let's look at how tatoos being "tattooed" on human body..
they use a special needle to paint the artwork on the skin

when we talk about needle..
it's a nature of the needle to prick
and of course ,during the process of tattooing ...
the needle pricked the skin a lot..
contacted with the fat tissue.. and also small vessels
meaning.. anything from the needle.. could go inside the vessel

so , in this case..
one sunday morning..
a HIV person went to the tattoo shop
and got a tattoo on his ass..

and the next person came in into the same shop
let's say some australian tourist he is..
got a tattoo as well...

and apparently the tattoo master use the same needle as he used for the hiv infected customer
and he forgot to clean and wash the needle thoroughly ...

there was a big chance.. the second customer could get the HIV as well
easily infected from the unsterilized needle
the same one used on the HIV patient

well ,
if that the case
he's such an unfortunately guy

moral of the story , dont get any tattoo...

but then.. same scenario also could happen at the barber shop or the sallon
when the barber use the same blade for shaving...

something to think about...

Working hard to lose weight

It is hard to lose weight when u have a big appetite and live with ur mother!

Currently staying with my parent while my wife is still her confinement period.
It's a good thing for me and my wife
But not so good for my belly!

Coz there will always be plenty of food on the dinner table at every moment!

U know every mom loves to feed their child. So am I. Whenever nia is crying... I feed her milk.
So everything I went into the kitchen or pass through the kitchen , my mom thought that I am hungry. Hence she starts cooking all sort of food..
Me ended up eating like there is no tomorrow
The final result.... Obesity
Not something to be proud of indeed!

Trying hard to lose weight.
Went to the gym everyday
But still not enough to lose the excess fat!
Damn...

HIV mother

had a busy oncall last night at work

i was in charge of the obstetric OT

giving spinal and general anaesthesia for c section operation..

a lot of emergency cases being posted after midnite

not something i was looking for

coz honestly ,

after midnite

half of our brain has already shut down

and people become a bit cranky when they stay awake after their usual sleeping hours

occay , nothing new there

just another day at work...

at 4 am , came a case of HIV mother , already in labour

for my readers info ,

HIV mothers are not allowed to go thru normal delivery

because we want to reduce the risk of mother to baby infection

all HIV mothers must be operated to make sure there is some chance for the baby to be HIV free

plus , all HIV mother must not breastfeed their baby as well...

so my patient , still a teenager

already has one baby when she was 14 years old

and she came for her second baby with her second husband

she claimed she got the HIV from her first husband... when she was 13

since then she was on medication for her HIV

nevertheless , for her 1st baby 4 years ago.

the baby was delivered manually...

hence , we were not sure of the status of her 1st baby...

could be a HIV positive ... and hopefully not

it was a sad story ..

and crazy as well

not sure about her second husband HIV status

but her husband claimed to be HIV negative

which is impossible

then before the operation started

i popped the big question to the couple..

do u still want more children after this????

u are HIV positive... and giving birth to another children..

well .... not the best idea

since u are struggling with ur disease

how are u gonna take care of the baby

what if ur baby turned out to be HIV positive as well?

pity the baby right?

we gave all the meds available on earth to make sure the baby delivered last night not infected

but the risk is still there

and we would not know the result of the baby HIV status immediately...

it's such a complicated situation

SHOULD people who are HIV positive conceived babies?

what will happened to their baby?

they cant breastfeed them

and when the baby grows up .. what would he or she felt , when he found out he got HIV from their parent...

God goodness..

this is beyond my imagination

Best ever anniversary gift

They said that diamond and gold are women best friends
I used to think that was crap and women love other things than those bling bling thingy
Nonetheless as I grow older and more mature
I come to a conclusion that the saying is so damn true
Nothing has changed ever since

Gold and diamond always bring smile to the ladies... Big big smile of course...
Am I right dear ladies? Hehehhe

Hence she got hers during our anniversary .
It s a token of my appreciation of what she has done for me..
A way of saying " thank you for being my wife "
Some said action speak louder than words
In my case , gold definitely speaks the loudest .

Then , mommy asked daddy
" what do u want for ur anniversary present daddy? "

Daddy said " don't worry, I already got mine.... She is currently pooping on daddy.... Nia is the best gift ever ! , thank you mommy :) "

Nia got injected!

We went to the clinic yesterday for nia immunization .
My goodness, there were so many babies there..
The waiting line was agonizingly long..

Worse case scenario... The doctor was on leave as well... Can't blame her... She is the only doctor at the clinic and she needs rest as well

Luckily , I know the clinic very well as I was supposed to be transferred there previously, but one way or another... I decided not to go...
Long story short... The staffs also knows me very well..
In view of the long que, mommy herself injected nia while daddy was holding her tight

We took preventive measure to make sure that nia was pain free during the injection.... Remember that I am an anaesthetist...pretty good at dealing with pain...

Nia was hardly crying during the injection... Just one tiny scream and she was ok already.... Thank you Allah... After the injection... She was doing well... No crying.... And until this morning ... Alhamdulillah... No fever.....

Going out for immunization at the local clinic....
And also.... Today is our anniversary ..

is dealing with patient who believe hospital is the last resort

many patients, mostly the malay ethnic
more commonly in kelantan
prefer to seek treatment from bomoh whenever they fall sick
or something happened to their body

case and point happened to me at the clinic today

a makcik brought her 4 month old baby
with impetigo , and infected wound all over the body
something like the photo above

me : mak cik , when does it started to become like this?
mak cik ; eh.... baru saje.... semalam baru makcik perasan..
me : mak cik , please dont lie to me , i am a doctor.. i know what i see
mak cik : dah lama dah sebenarnya... dah dekat sebulan... mula sket jer.. pastu makin teruk

me : then , why dont u come to the doctor earlier? x kesian kat anak u ke dah jadi mcm ni...
mak cik : mak cik pergi jumpe bomoh... bomoh kasi air tawar... lepas seminggu 2 makin teruk baru mak cik bawak datang sini...
me : kesian anak mak cik ni... dah teruk sgt. .. dah kene jumpe pakar kulit... x tau bole pulih ke x nanti ...
mak cik ; tu la mak cik bawak dtg sini nak ambik ubat la...
me : apsal mak cik x pergi follow up dengan bomoh jer? bomoh pakar xde ke?

well, this is the attitude of our people
still believe in bomoh
still believe that bad health caused by the black spirit
for crying out loud

please come to the doctor first,
then if we cannot treat u ...
then go find ur bomoh
not otherwise

the best part of this cases..
when these people come to the doctors at the terminal stage of the disease
some of them , died at the hospital
then these people started to blame doctors for their relative ill fated.
ridiculous indeed!

Last month I wanted to sell my red bumble bee...

The reasons?...
None of the is valid

After a long thought....
I decided not to sell it
This is my first car ,
And I love it...
It is a part of me
It is associated with me
Some people knows me coz I was driving this car
I made so many friend in the car club
I had so much fun with the car...
It is something priceless
Hence... I am keeping the car

I had a few good offers for car
Some were too good to refuse
But everytime when I meet with the buyer and people are test driving my car...
I felt a part of me was taken away

Some said, it was not a smart decision to keep the car
A car is a liability , not an asset
It need maintainance ...
Regularly
And it costs a lot of money...

So what..
I have money ( not that much actually)
I love my car
I am loyal to it

Open letter to mommy nia

Dear mommy nia,

Daddy knows that nia is at not his best behavior right now
She has been crying a lot..
Keep u and mama awake all night long...

Daddy knows that mommy has been working hard to take care of nia
Daddy knows that mommy is doing her best to make sure nia grows up healthy

Mommy has been pumping her breast milk 24/7 to make sure nia gets the best milk in the world.

Daddy also knows sometimes mommy felt very tired
And frustrated at times as nia keeps crying non stop
Mommy has been doing superb job as a mother so far and daddy really appreciate that

Daddy knows that daddy has not been around a lot
Daddy is working hard for our family
Make sure that mommy and nia get the best in life

Once nia is old enough to travel around... We will go a wonderful holiday .

my nephew is only 28 days old..
he is active..
mom breastfed him well and he drink a lot..
sometimes he burp easily but sometimes it takes time for him to burp...
and he poo a lot too..
about 5 times a day...

the problem right now is he will cry damn loud in midnight untill 3/4 in the morning..

i ve saw I LOVE YOU massage and a few more technique to reduce the gas. but, it doesn't help a lot..

we had brought him to the clinic 2 days back..and the doctor said the baby is having COLIC.. a lot of gas in his stomach..
the dr gave a red colour liquid medicine to be taken 2.5 ml, 4 times a day..

as a result, the baby is soo sleepy..
is it fine for him?

and yesterday we try to reduce the dossage to only 1.5ml...but he cried out loud again in the midnight..

he is doing fine in the morning..

is it colic was caused by the antibiotic gaved to mother when the amnion liquid burst 24 hours before delivery?

u r not alone
Nia also has the same problem

she has colic as well ,
the abdomen become distended
and she is always awake after midnite until early morning
no new issue here
her mommy and granny are also busy trying to keep her calm and asleep

this issue is very common among infant
nothing new here
but it's very tiring for parent
and very frustrating as well

there are a few solution for this problem
first , u can go to the doctor
and they will prescribe u with GRIPE WATER
it's safe to use for infact to reduce gas
after taking this syrup ,
the baby will fart , a lot

hence reduce the gas inside the stomach

however, should ur child still having this issue despite this medication
u should bring ur child to the pediatrician for further assessment

in the market currently
there are also special feeding bottle that can avoid colic
nevertheless, the effectiveness of these bottle are pretty much questionable

occay , those are my answer as a doctor

should u ask my mom.
she will advise u to "mendemah" ur baby

and use the sand inside the cloth to slowly massage nia's stomach

when i was in charged of the labour room few years back..
the labour was busier than the bloody factory

bak kata my friend from terengganu who was my fellow houseman back then..

we need new generation
these new generation will lead the world someday
and we are the one responsible to produce future leader..

not all babies have the best care from the mother..

when i said best care , i am not talking about baby court made from gold..
best care from parent meaning... exclusive breast feeding or breast milk , on time vaccination... and most important ... love and care from mom and dad...

nevertheless , most of the young parent nowadays already failed at the first part..
the breast feeding
i do understand not all young and new mother can produce enough milk for the babies
and some babies do have trouble with sucking
hence making breast feeding almost impossible

however , we live in the 21 th century with all kind of technology and endless list of drugs
there are so many medicine on the market that can increase lactation for those who produce little milk
for babies who has problem with sucking the nipple
the mother could just pump out the milk and feed the baby using the regular bottle
yes, the intimacy of breastfeeding may not be there but at least the baby is getting mother's milk

there is no excuse nowadays for not feeding ur babies
unless u are a HIV patients , then we understood why u r not giving ur milk to ur babies...

i am a father , and i want my princess nia to be fed with breast milk
hence she will grow up and be as pretty and as smart as her mother

unfortunately nia is not a big fan of breast feeding hence my wife keep pumping the milk everyday..
as long as nia getting her mom milk everyday, it's more than enough :)

i know i may not be the first doctor who is going to write about this

there were already so many open letter , articles

written by medical officers, specialist

regarding the current issue of the houseman'

also know as the internship .. or new doctors

the new rules implemented by the government says that houseman must not be overwork

they must not work more than 12 hour per day and must have 2 days off every week

no oncall for houseman

then , they also must not be scolded and must be pampered ( occay , i added this part )

well , good for them.

when i was a houseman not long ago , i work every day .... maybe 4 days off in a month at most ,

everyday we work more than 12 hours ... if we were to be on call.. we work 36 hours non stop

yes .... if u compared the condition during my time and current condition ... it's a lot of different

in a way , it's an improvement

in a lot of way , it's bull shit and a lot of crap

like the proverb above said.. doctor must work more than the current houseman rate

i dont know who decided to give this houseman such a ridiculous schedule

but in the end of the day , it is not a way to teach houseman to be a good doctor

to be a good doctor , we need a lot of training

to have a lot of training ... we need to spend a lot of time treating patient

to spend a lot of time treating patient

we need to stay extra hours at the hospital

12 hours per day is not enough

currently the houseman works in shift system , am is 8 am to 8 pm... pm is 8pm to 8am...

sound very systematic right...

yes ... if u work in the office dealing with document it's fine

but not for doctors

we doctors have CME everyday , a place and time where we learn new things every morning .. so , those who come in PM shift.. they will miss the CME... how do u expect houseman to learn?

then , for those AM shift houseman ... they supposed to work from 8 am , to 8 pm... but

generally in hospital ( unless in the emergency department )

from 5 pm to 7 pm is visiting hours and during that time , doctors dont do round ...it's time for rest... hence the houseman end up doing nothing from 5 pm till 8 pm....

might as well allow them to go back by 5 pm right?

this is logic thinking

then.. for those who work from 8 pm to 8 am...

they actually work from 8pm till 11 or 12 at most...

by midnite.. everyone fall asleep ...

eventually learn nothing for the rest of the shift

i wonder those who suggested the shift system ever work in the ward :p

i'm sorry to say , but i strongly disagree with the shift system

it's such an disadvantage for those houseman who wants to learn more

for those who want to relax , it's a golden opportunity

with this kind of system.... anybody could be a bloody doctor nowdays

imagine ur family member being treated by these unskilled doctors?

back in my time ... we got scolded by medical officer and specialist badly if we made mistake and being screw upside down if we do wrongly..

Yes , in kelantan we have Sunday blues instead of Monday

It was raining heavily this morning...
On my way to work, the traffic was very slow...
Yeah, I know it is raining but doesn't mean u have to drive at 10km/h speed . It was killing me!

Worse case.... Those slow drivers were driving on the right lane. Don't these people ever saw the sign .... Laluan kanan hanya untuk memotong... Keep left if u r not overtaking... Stay on the left lane if u r so freaking slow...
Instead these tortoises choose to stay on the fast lane... And cause more traffic... Bloody hell..

Seriously Dato hishamudin... If those who drive fast can be punished... Those who drove slowly on the right lane should be as well....

Well.., this issue has been bugging me every single morning.... How I wish I could just slap a big sticker on those slow driver windscreen... " move u ..... Ttuttttt"

granny : salam... this baby is not well , maybe having fever .... she is 3 month old..
me : owh occay.. let me take a look ....does she has running nose?
granny : yes.. that too..
me : very well ... how about feeding? where is the mother? does she breast feed her baby?
granny : the mother is away ... the baby was never breast fed ...
me : mmm.. that explain a lot... may i know where is the mother? is she working?
granny : no , the mother is not working.... she went to KL to attend some birthday party with frens.. hence leaving the baby with us to take care of...

owh my God... for crying out loud! ( well, i didnt said this actually )

Me : mak cik... the baby need her mom milk... the milk which contain all the vitamin and antibiotic which will keep the baby healthy... no drugs can replace the mother's milk... why does the mother refuse to breast milk?

granny : die malas... die kata susah nak susu badan..

u mentioned that ur friend is already at the stage ll
it's still an early stage
but we both not sure about the severity of her condition right
usually at stage ll
a patient could still have an active daily life
but the symptom occurs quite oftenly
in her case , symptoms could be severe headache and mood swing

in many case , at stage ll of cancer,
combination of surgery and chemotheraphy do produced a good result
nevertheless , these two methods also come with bad side effect
in her case ,
if surgical to be done , maybe a part of the brain need to be removed
and during chemotheraphy ...
other healthy brain cells will be dead as well resulting for the toxic effect of the chemo drugs
imagine if u only have half of ur brain
that's is one of the possibilities that could happen to her

we could treat the cancer ,
but her brain would be effected as well...

she could fight until the end... but it would be a difficult battle to win...

writing about smoking reminded me of one of my patient who got lung cancer after being a chronic heavy smoker for many years

as an anaesthetist , we are also in charge of the acute pain service management
we are the people to be called when the doctor cannot treat the pain
we are pain specialist , so called - equipped with thousand types of pain killer..

so , there was this patient
he is already at the 4th stage of his cancer
no surgery of chemotheraphy could cure his illness anymore
technically , he was just waiting for his time to go

for everyone knowledge , cancer is a very painful disease
when u are at the terminal stage of the cancer
u will fell pain all over ur body
the pain was excruciating...
no amount of pain killer could heal the pain

same scenario with this patient..
even regular intake of morphine could not make him sleep well
there was time he was bagging us to take his life
but we are no God

i told him , " sir, we may not take ur pain away , but we are doing our best to make u feel as comfortable as possible "
we started him on the fentanyl and ketamine infusion ...
the highest level of pain killer available on earth..
and yet... he was still in pain...

not long after that ... he passed away...
i just hope ... what we did for him.. could at least heal a bit of his pain...
maybe not much...
but at least... he knew that we did our best...

electronic cigarette has become very famous lately
the manufacturer claimed that electronic cigarette or also known as e-cigar is less dangerous compared to the normal cigarette and it's also a method to stop smoking.

well , that's what the manufacturer said they also firmly stated that their e-cigar does not contain the dangerous chemical such as the carcinogen which is found in the traditional cigarette
again... that's what they claimed of course they are putting good word for it coz they want to make money

nevertheless... WHO said otherwise
WHO said , e-cigar is harmful as the normal cigarette studies have been done and showed that e-cigar also contained carcinogen which can cause cancer

well , that should sum it up...
it's not a solution to smoking after all the only good thing about e-cigar is , it's less smelly and less smoky

the rest - only bring more harm - it's more expensive and in a way , it does encourage people to start smoking as they said e-cigar is not a real cigarette
so,

my advice say no to any kind of smoking , e-cigarette , normal cigarette , shisha - all of there will only lead u to the palliative ward eventually

* palliative ward is ward for people with cancer which cant be treated!

I play golf... Not professionally
But always bring my golf club along with me should there is chance to hit the green..

Just saw the news at tv3 bulletin utama, straight away went to check the detail in the net.

The supercars convoy led by a 60 years old pakcik... Were speeding on the wet highway... Until the pakcik suddenly spun his car... And the other followed. Including one lucky Prius . Yes, the Prius is also counted! Even thou obviously it did not a part of the convoy .

Many of us have received this SMS. It has been circulating around for the past few days.
Nobody knows how true is this rumors , who started it.... And if it is true , I still havent heard no official press conference by the manufacture yet.

Occay, some said , whether it is true or not... Why take the risk right? Plus carbonated drinks is not healthy anyway.

Clinically and physically, it is quite impossible to be infected with HIV via oral route.

Even if u had an oral sex with an infected person... U hardly get the virus... Just an example.

Technically, to able to infect everyone via oral route ; iin this case putting an infected blood into the drink... It require a large amount of blood ...
I don't think a person has that much blood to fool around with.
Unless he committed suicide by jumping into the water tank of the company... And the body rotted inside it...

this guy , khairul fahmi a.k.a apek has become the most searched people in the internet lately
and he surely made the headlines at the newspaper everyday
for good reason , as well as all the wrong reason

the good part , as we all know , he's our hero - just being announced as the best malaysian goalkeeper
and he definitely was the best player during the recent sea games

unfortunately for him - he's also pretty much famous for the wrong reason as well currently
accused of being scandalous with a few pretty girls
and some of their intimate photos being spread all over the net world

pity him
he's pretty much stressed rite now , and voiced out the fact he might be retiring soon due to the crazy rumour being going him lately.

Hanania aqiqah was held last friday , after the prayer ,

we were very grateful that the imam n bilal were able to attend the ceremony

Hanania shakira bt mohd hannan ,

hanania means - being loved , shakira means being thankful

such a beautiful name for a beautiful baby girl

according to our beloved prophet S.A.W , it's best the baby being name at the seventh day of her or his life. hence Hanania name only being announced during the day of the aqiqah which is her 7th day of life

during the aqiqah ceremony , being led by the imam - prayers being read , and she was fed with a sip of zam zam water and a little taste of kurma

then , here comes " bercukur jambul" part , it was my first experience ... ehehhee.

then the baby being paraded around the crowd while selawat being sang...

so glad to be back home to cuddle with my little princess

when i entered the house,
i could hear my little honeybunch was crying aloud
i went straight inside my room to take a glance of her...
she was awake , mommy was getting ready to breast fed her...
a bit cranky... maybe she was hungry.. or was very sleepy...

i took her into my arms and repeatedly saying " baby nia... daddy is back for you sayang"
she looked me into the eyes..
her eyes shine brightly...
that - took away all my problem ...all the fatigue was gone in a split second...
holding her in my arms was the most powerful energy booster
way better than red bull.

i sang her lullaby... and she slowly felt asleep in my arms
such a wonderful feeling....

The hardest part of my morning today was saying good bye to my little precious princess.
My paternal leave has ended and it is time to go back to work
Thank to the 7 days off, I was able to spend my days watching my baby grows up very second. Such a great moment.

Now, leaving her at Home with my lovely wife
Hopefully she will behave well... Not causing much trouble for mommy

After I shaved her head, she does become a bit cranky,
Maybe still not used to the fact she is pretty much bald!
So yes, for the past 24 hours, she is on " angry bird" mode...
Or worst case scenario.... Is having PMS already.

Regardless of everything.. She will always be my little princess...
Hanania... Daddy loves u much...
And of course... Daddy loves u too mommy

Now daddy has to work hard so my queen and my princess could have their dream castle soon enough

occay , to start with , for general knowledge , lipoma is a kind of benign tumour (mostly ) , made from adipose tissue , in malay , ketumbuhan yang berasal daripada lemak-lemak
lipoma is a painless mass , located subcutaneously.
in an ideal world , this lipoma could be easily be removed surgically.
no big deal
if it's a small size , the operation could be done under local anaesthesia , meaning , u will be fully concious during the procedure - only the area involved being anaesthetized... in most cases - u could still feel the pain.
but it's the quickest way
should the lipoma is too big , and need to be removed , it must be done under general anaesthesia , meaning .. u ll be put to sleep - completely knocked down during the procedure.
now , we go to complication of the removal of the lipoma
most of the cases - it has no complication
just a very simple procedure, something like removing a big pimple from ur skin
unless the lipoma is super big... then there would be risk of bleeding during the operation
other than that - normal sized lipoma , technically it would not cause any damage to ur spine mr shamsul
effect on ur neck?
u might have a scar after the removal...small or big.. again depend on the size of the lipoma :)
hopefully , i have answered all ur question .
owh , one more thing , when they excise ur lipoma , they will send the sample to the lab , to determine what kind of lipoma is it.
but most of the time , the lipoma is harmless
unless u are a very unlucky guy , something happened once in a million cases. .. it could be some kind of cancer...
No worry , i m pretty sure urs is just a benign lipoma :)

as u all know
i am a big fan anti smoking
i hate cigarette
i hate people selling cigarette
and on top of everything i hate people who smoke cigarette

but , worse case scenario ..
is parent who smoke cigarettes
could be the daddy.. or the mother..
either way, it's still wrong

unfortunately for us here in malaysia
it's such a common scenery in our daily life
we see parent walking hands to hands with their children
which is a wonderful thing
then they stopped in front of a shop ..
the kids went into the shop with the mother
the father will wait outside
and quickly reaching something in his pocket..
a box of cigarette and a lighter
and he'll start smoking while waiting for his family to come out from the shop..

after a few mins , the mother and kids emerged from the shop
to join the father outside
then they all continue walking hands in hands again..
as their father peacefully smoking his cigarette using his other hand...
without knowing how much damage he's causing to his children walking next to him...
what a shame
this kind of father should be sentenced to jail...
seriously..
such an ugly situation..
u right hand is holding ur kid, ur left hand is holding the cigarette...
u might just as well bring along a bottle of paraquat...
it's killing me to see this kind of parent

even if u cant stop smoking
the least u can do is stop smoking in front or nearby ur children

didnt realize i was speeding so fast on my way to the market to by coconut milk

can ur kancil do this - running at 160km/h ... and the RPM is only 3000 round per minute

It was about 2 guys who wished they could have each other's life instead of their current ongoing life

gegirl - day 3 of life

look at her cute face....
just being breast fed this morning
a bit jaundice as her mother still struggling to produce enough milk
but i am pretty convince she'll be ok when more breast milk available

parenting - so far it has been very tiring for both of us
last night again - baby girl kept us busy
she was ok during day time...
was sleeping well
but after midnite - here comes the drama

i reckon she has a bit abdominal discomfort
bowel opening becomes so frequent last night
more breast milk should fix the problem

since the mother has not been producing enough breast milk
we opted for soy based formula milk for stand in replacement
until the breast milk production increase

nevertheless - despite all the sleepless night for both of us
we could nt care less
all we want is our baby girl turn out healthy and beautiful in the end of the day

day 2 of life
my beloved princess... my gegirl..

well , i guess at some point of my life , this blog will change its name to dr hannan fatherhood

thank you for all the warm wishes guy,
then comes the next fav question
"how's life as a father "
well... so far.. hhhmmmm
pretty much indescribable

but on top of everything.. i feel very happy and very much excited

yes - it's tiring as well
truth was... for the past 72 hours...
i reckon i slept less than 10 hours

a day before my wife went to labour ...
i was oncall.. hence.. there was very little sleep there
the next day , i went back home , rested for a while,
then when to the gym in the evening...
after that , while i was at the clinic at night ...
she went into active phase of labour..

from the clinic , i rushed to the labour room
it was about 11pm when she was admitted to the labour room.
gegirl was born at 340 am the next day ...
and i was awake thru out the processes

6 hours later , we went back home.
i was thankful everything went well
and my baby and my beloved wife were healthy and doing great along the way

at home... last nite my gegirl was sleeping well during day time
but after midnite .... she kept us busy until dawn...
since mommy still struggling with breastfeeding...
she was pretty much hungry last night .. hence
we both didnt much rest or sleep

nevertheless
things are getting better today
gegirl is feeding well
less crying
but she's very active :)
let's see how she's reacting tonite yeah?

mmmmmmmmmm...
WHAT's her name?
it will be keep as a secret until her 7th day of life

the delivery went well with the help of epidural
thank you to Dr Razita for the epidural
and staff nurse Lim for assisting me delivering my own baby

such a wonderful day for both of us
well.. not both anymore... now it's three of us

weighing 2.8 kg at birth
she brought such a joy and happiness to the family
she took away all the pain
she makes me thinking of home every single second
(no offence my darling wife - u have the same aura too ! )

it's been bugging me

had a hectic day at work so far today
very problematic cases today
i wish i am not in charge of the Operating room today...
unfortunately .. i am

nonetheless... everything still turns out fine eventually
above everything... i hope and pray there will be no mortality

i love being a doctor
currently i am a part of the anaesthetist team
it's the same like special task force team in the police department
or the komandos in the army
we deal with life and death situation everyday
we r in charge of the ICU
we r in charge of the operating theater
we put people to sleep ... and we have to wake them up as well
it's a very demanding job...
and it's killing me

i love saving life
it's a noble job
but then.
sometimes... i feel so tired
and stressful....
i wish i could be in charged of something more stationary
like a klinik kesihatan
where i work 8 to 5 everyday
it's a good life
yes... there are death cases in klinik kesihatan as well
but only once in the blue moon.

nonetheless , there is something special that keep me staying at the anaesthesia department
the people ...
the bosses ... my collague ... they are the best people ,
the nicest group of human being u want to be around with
they cheered u up when u r down
they stand by u when u screwed up
the helping there is always next to u
it felt like a family in the anaesthesia department
unlike other department i work before
that's the main reason that keep me from leaving the hospital...

yet and still...
i have my doubt..
despite of all great people around me..
am i able to be a good anaesthetist,,
and how long could i hold my breath here....

i am selling my beloved mitsubishi lancer GT
yes - need something smaller
something more fuel economic
in these hard time , need to save as much as possible
and it's impossible for me to travel without style!

hence , i am having a hard time choosing
which one will be my first hot hatch

the cheapest would be the ford fiesta ... around 80k...
pretty looking- sexy and reasonable price
but there are too many of them on the road now
i love the sapphire version but they are not selling those anymore
it would be the perfect choice - but it's very common nowdays

then comes the newly launched hybrid car - honda CRZ
hybrid - meaning great fuel saving,
price is about 115k on the road - which is cheaper than i expected it to be...
but then...the downside is..
currently in malaysia - honda is selling manual transmission
which is not very convinient to my other half...
so... probably no for crz until they bring the auto version

3rd choice .... vw polo GTI
great car.. super fast hatch back...
the fastest among all..
the most handsome looking...
superb built quality...
price- 138 k.... still affordable..
but - no vw official dealer in kelantan
meaning... no vw service center
meaning ... i have to drive to KL to service it...
which ... i dont really have time for that...
what a shame... come on vw... open ur 3S in kelantan pls....

this is the second case of the year i stumbled upon in the ICU.
it irked me a lot.

DEAR citizen.... if u have HIV - do tell ur doctors , who treat you . it will our life way a lot easier and better.

A study show , in a profesional life of doctors who deal with patients everyday , the risk of getting HIV from the patients is about 1 percent.
Meaning.... if a doctor treats 100 HIV patients ... he's pretty much unlucky if he's not HIV positive as well.

Yeah , we must treat all patient equally ...
but doctors are human too, we are not bullet prove of AIDS.
we could easily being infected by patients.
we took blood everyday , we operate on patients everyday

imagine , if those people who has HIV , keep their HIV status as a secret...
and one unlucky day ... a doctor accidently had a needle prick incident with that patient...
how would u feel if that doctor is a member of ur family.

we treat people
but SHIT people are HIV positive and did nt tell us... make our job so dangerous!

worse ... it's a waste a money to treat HIV positive patient
they are pretty much immune depressed... all the drugs in the world can heal them...

u being a HIV positive patient is enough problem for us...

while i was anxiously waiting for the birth of my baby .... anytime soon...

asking me about abortion

i promised her that i wont tell anyone about it
nevertheless - i am writing this issue , generally...
not mentioning her name
and nothing about her...

i just want people to be crystal clear about abortion.
i believe this is a very important issue....
and believe it or not... abortion is quite common in malaysia
worse case... among Malay which are muslims.
what a shameful true fact.....

firstly - being a muslim... Islam said abortion is haram....
meaning... if u aborted ur baby intentionally... technically u will end up in hell.
as simple as that
unless being pregnant is dangerous for the mother, then - abortion is permitted
( example - mother with severe heart problem )

secondly - as a doctor , we are not permitted to do abortion for social reason
we are trained to do abortion
but only for medical causes
those doctors who do illegal abortion - they could be charged and their certificate could be rebuffed

yet and still , there are many muslims out there who do illegal abortion for social reason
and sadly , there are doctors whom morally degraded - willing to do abortion....
i call those doctors stupid assholes and jerk....
how i wish the government could charge those who do illegal abortion with death penalty!!!

there are so many social reason given by the mother , whom decided for abortion....
most common .. those unmarried pregnant ladies with " anak luar nikah "
then , there are also young mother whom are married ... but not ready to be a mother yet...
all these reason are bull shit...

frankly , i am not into politics. for crying out loud , i wish i could stay away from politic. but i cant , it's a part of everyone life

it was a jerk reflex situation
but i do have my reason

had a little situation at the pharmacy earlier today
not something i am proud of
since it happened in front of a lot of people
well... those people actually agreed with me...

i was taking a few medicine for my beloved granny
whose having problem with her heart..
and she was on the medication for like forever

so today i went to the pharmacy to take the meds as usual
not for the first time..

i went in , and gave the medication slip to the person in charged at the counter

apparently , according to the slip , it written there i should be collecting the medication
a week earlier ...
and here the problem started

the lady said - i should be collecting the meds exactly on time
should i miss the bloody date
i need a new slips

and then ... i politely said " it was just a simple issue... a week is not such a big issue "

then the lady replied " i am sorry sir ... ( not realising i am a doctor ) ... the rules said so... u missed the date u need to get a new slip "

that got my blood boiling ...

" please be reasonable ... i am a doctor .. i am a staff here ... please be considerate ... i could have go and write another slip for the meds , but i dont have the luxury to go walking around the hospital as i have a lot of patient to attend " i replied

but she still insisting not giving me the meds ..

" call ur superior " i said

then came one older lady ... telling me same answer... NO.. and she was nt even a pharmacist ... just an assistant..

it makes me so pissed of at that very moment

" u know what lady.... yesterday ... one of ur staff brother had an accident ... and he suffered so many injury and required icu management and ventilator ... however no ICU bed available at time in our hospital... and u know what i do for ur STAFF? I arranged him to be transferred to KPJ perdana - all FREE - , even thou i could have let him stay and died in the ward since we have no icu available...
and u here...( being so SUPERIOR even thou ur grade is far lower than me - i didnt say this part )
is making a fuss about me being late for a bloody week? "

and she was all red ... and i am sorry all other patients waiting at the pharmacy has to hear that

she quickly get the meds for me ..
and i said thank you

well..
in the end of the day...
we all are hospital staff
our main priority is patient's life
my granny is also a patient
so does the young man i sent to KPJ perdana
i did my best for my patient
and i supposed she could be more sensible .....

my medical leave is coming to the end
i went for tonsilectomy about a week ago
it was an elective procedure
something ..... i just do to improve my quality of life

my tonsil has been treating me very bad lately
it has been infected for a few times
and been admitted twice for acute tonsilitis

those were awful situation
and eventually
i decided to remove it
hence, no more tonsilitis if i have no tonsil
yeah...
sound very easy
throw it away ,
and i ll get a better life

some said.. " if it is not broken... no need to fix it "
in my case... it was half broken
i could live with my tonsil intact
but it will cause me more trouble in the future
after a long discussion with myself....
... tonsilectomy was my final decision....

HOW was the operation?
well... it went well
not much complication
but the post operation pain was very severe
it hurt so much
the first thing i said as soon as i woke up from the anaesthesia...
" owh my God... i wouldnt go for the operation if i knew it hurt this much"
and the surgeon was laughing out loud .... while saying... " i told u so...."

yes.... i heard , and i knew tonsilectomy is very much hurtful
worse in adult
but i never thought it was that hurtful...
it was such a scary situation for me
such a horrible condition post op..
the pain was very severe.....

sigh,,,

tonsilitis - part 1

i was unwell
my tonsil was once again being infected by crazy bacteria
story of my life

i have been sick for quite few times this year
all , thanks to my beloved tonsil,
the bloody culprit

until this time - i didnt realize the tonsil was the mastermind
for all my unwell being lately

i reckon i got the infection while i was doing locum
yeah ,
at hospital
we use full protection ,
mask ... scrubs , hands sanitizer ...
all those thing which keep the bacteria and virus away
but then
during locum
i dont really wear a mask
since patient who pay to see the doctor
most of them would love to see the doctor's face

so , during my previous locum
i have a few patients with urti
some quite severe
and i reckon... there was the source of my acute on chronic tonsilitis

how do u know u have tonsilitis?
well ,... to start with... u have to have a pair of tonsil! ( lots of kid had them removed during school age )

it started with the symptoms..
sore throat....
then u started to have fever...
in my case... very bad fever .. my temperature went up to 39.5 degree celcius...
and accompanied by severe chills and rigor....
not to be forgotten... the common feature such as body weakness and headache...

worst case scenario happened to me...
i was oncall the night when my temperature was at 39.5 ...
and i was having crazy chills and rigor inside the ot that time...

as the condition worsening..
u could feel something sitting inside ur upper respiratory tract
something big... and it caused a lot of pain in ur throat
i would say , it felt like having a cactus at the base of ur tongue
yeah... exactly felt that way..

then,..
u will have difficulty in swallowing food...
which eventually cause u dehydrated..
u lips become dry... so does ur skin
as u take orally less , since the throat has obstruction in it....

i was in dilemma

and again...
a long absence from this blogging world...

but still.. i am back
still in raya mood
wishing everyone a happy hari raya

truth was -
i was in dilemma
i have been thinking hard lately...
last few days
consulting a lot of people

and i reckon i have my decision
a right one i hope

i have been working at the hospital ( GH) for quite sometimes
and i am telling u ,
i'm grateful and happy to be able to serve the people
it was my honour to treat the unfortunate
it was a wonderful moments to save plenty of lifes
and i was very glad

nevertheless
all of those - it is very tiring
and very exhausting

i want to take a rest for a while from all these adrenaline rush
i want to take a break..
being an anaesthetist in such a busy hospital...
is taking its toll on me.

maybe it's time for me to go somewhere else
somewhere more quite
somewhere i can work with not so dying patient
somewhere where i dont have to see people die everyday.

i have decided to ask for transfer.

and i know...
my anaesthetist knowledge and skill will be go to waste once i work at small clinic
but i need a fresh air

working on aidil fitri

ampun maaf di pinta
zahir dan batin.

it has been a wonderful 29 days for fasting
and now it's time for celebration
a day of joy and togetherness

for most people , yes.
but for some
they do have to work
i was lucky , having a lot of non muslim working in the same department
hence all the muslim could have day off during aidil fitri

still, there are many muslims out there who has to work during this blessful day
in a perfect world , i wish i could shut down the hospital during hari raya hence
all the staff could celebrate hari raya with their beloved family
nevertheless - we live in a not so perfect situation
there are sick patients everyday
there are people who need emergency surgery while others are performing raya prayer
there are unfortunate people who need icu care while others are eating lemang and ketupat

for all of these situation
a lof of people need to sacrifice their happiness
to save other people life
this is happening for not only hospital staff
same goes to policemen , fire fighters and others.

and yes - for all this sacrifices - at least a patient could do , or the relative could do is - say thank you

- which rarely said by them
and we dont really expecting those actually
but for goodness sake
at least - do not complain much and being very fussy during this day

i had experience the situation...
and a lot of my work mate encountered the same problems too...
the patients n relatives been complaining a lot
especially during hari raya...
someone is sick ,
the whole family went to emergency department
and everyone start complaining when the doctor didnt attend them immediately

please understand during hari raya - we dont have enough staff
only oncall doctors are working
same goes to staff nurses and medical assistants
hence , for non life threatening cases
u may not be attended ASAP if there are other near death situation around
the least u could do is say thank you to the doctor who left her or his family
to treat people during hari raya.....

had a busy , hectic oncall yesterday
i am exhausted
i am JADED....

i'm drained - physically and mentally.
last night - the cases keep coming - non stop
the icing of the cake was
we had a 12 hours surgery
involving many dicipline

a gardener was involved in a very serious moto-vehicle accident
and he suffered a heavy injury ..
multiple fracture..
basically , the face was torned apart
the facial bones are scattered ...

so yes , he is in a very ill condition
and last night he endured a very long surgery to fix all the fracture
involved so many dicipline

we started with ophtalmology team to fix the wound at the retina...
then came in the orthopedic team to deal with the arms...
the wound was very dirty and the surgeons was struggling to make sure the wound wont be infected in the future...
those two teams took about 6 hours ...
the came in the dental team..
to fix the facial bones..
and my Goodness...
the surgeons needed 6 hours to plate the facial bones back together
not sure how many titanium plates were used last night
but i know there were a lot

and yes -a total of 12 hours surgery..
3 teams...
so many surgeon.
but the same pair of anaesthetist ..
we i have to say ..
we were very tired
trying to keep the poor gardener alive thru out the surgery
which was not an easy job i have to say
but - it was our job to make sure the patient survived the operation

alhamdulillah , the surgery went well..
the patient was ok ...
no immediate complication seen
he still need to be ventilated and intubated for few more days
for airways protection
and i believe he will come out well...

and for us ...
i dont know what happened to my partner this morning.
all i know ,
someone wake me up around 8 am ... i reckon it was my boss who came to do icu round..
still i couldnt open my eyes..
i only managed to hit the shower at 11 am....
woke up looking like a lion i believe..
took my shower... and headed home safely...

now... i need my well deserved rest..

being a doctor
we should know our limit
we should know our capability
we should be able to see the consequence
we must be prepared for the complication
we must be aware of side effect

nonetheless
some doctors ,
they think sky is the limit
they think they could fix everything
they think , their hands could cure all medical illness
they think , they could fix all the fractures

so much we could do as a doctor
but in the end , it's God who decide
God is fair
God always do justice
should we do something beyond crazy
then - complication awaiting

doctors - we consider 50-60 years old people as young people
and these people shouldnt die due to simple medical illness
i totally agree with that

but how about people who are 80 to 90 years old
personally these group of people are gifted
gifted by God with long life and good health
some people , at the age of 80 or 90
still doesnt have any medical illness
and technically they are fit

nonetheless - these group of people
they are very fragile
i simply disagree with a few doctors who are putting these people to do
elective surgeries
as an anaesthetist ...
we put patients to sleep as they being operated by surgeons
and i my little experience
to put a 90 years old patient to sleep to go for an elective surgery
it's killing me inside

if u are 90 years old
ur body isnt functioning at normal rate even thou u dont have any medical illness
hence , giving anaesthesia to this people ....
i would say - it's best to be avoid
if the surgery cant be postponed... or it can be treated conservatively , please do...
not many people in the age of 80 to 90 years old
came out from operation and live long after that...

a member of my family has just passed away
he got severe complication after an elective surgery..
he was 82 years old
al fatihah for him...
may he rest in peace.

someone did suggest me to replace my teeth with this cow's teech

but i do have problem when seeing dentist
i have limited mouth opening
it is very hard for me to keep my mouth open while people are putting stuff inside it

i remember vividly my experience with dentist
i went to my cousin
even she had a big issue dealing with me
i felt like vomiting each time she put the probe inside my mouth
i have a very good gag reflex i reckon
good for me , bad for dentist

next time i go for check up
i need to be put under General anaesthesia hence the dentist could do her job easily

anyway ,
raya is just around the corner
am sure everyone is so damn excited and sexcited

we are not very traditional when comes to food for hari raya
yes ... we do have sate , lemang , ketupat ... bla bla bla
but there will always be something special..

2 years ago we had nasi dagang with gulai burung puyuh..
last year we had a whole kambing .. panggang
this year... we have duck... yes.. duck..
so , everyone is invited to my house for hari raya
and of course it's limited...
first come last served! hhehehee.

on how do i managed to become a doctor at the age of 23...

well, nothing fancy , nothing amazing
just ran with the flow
a lot of my classmate managed to do so as well
here is the secret

i sat for my SPM at the age of 17 , same as other people
then by age of 18 , i got the offer to go oversea to study medicine
on my 18th birthday i flew abroad
started my first year ( pre med )
so , 1st year med - i was 19
2nd year med i was 20 ,
3rd year med school i was 21
4th year , i was 22
5th year - the final year i was 23

came back home to malaysia at the age of 23 , after a month resting at home...
then i got the offer to start my life as a houseman

next question.. what's the different between Houseman (HO ) and medical officer

a HO is actually not a registered doctor yet
just a training period
of 2 years
( yes , u do get ur UD41 grade as soon as u started - and a full UD41 salary as well )
in that training period - a houseman is protected by the hospital - should he did anything wrong
the law cant sent him to jail
anything he or she screwed up , the specialist in charge will be responsible
which explain why the houseman needs a lot of training

once u finished ur housemanship
then u get ur full registration certificate from the medical association
then u become a real doctor
u will be called the medical officer by that time (MO)
u will get ur UD 44 grade as soon as u get ur full registration certificate
which means - u are on your own
u are responsible for everything u did
nobody will protect u if u screw up

so there you go

owh my gucci!
such a horrible situation right now
i have toothache
and it's killing me

definitely worse than PMS ladies...
i dont know why
worst case scenario - i m allergic to a lot of painkiller...
which left me with a little choice to deal with my excruciating pain right now

it ruined my mood for the whole day
complicated with some issue in the OR today..
gosh...
i wish this pain will fly away

yeah i know... go see a dentist
in fact , my cousin next door is a dentist
need to see her tomorrow
but her help would be limited
coz my allergies to the drugs
bloody hell....
mmm...

no more daging panggang
no more grilled lamb
no more steak
no more heavy stuff until i am done with the pain
just plain rice and something soft

i do went for dental check up before
everything was fine
i do eat proper food
enough calcium i believe,
mmmmm

should i am not allergic to NSAID
the best drug would be voltaren / diclofenac / nurofen
as it ease the pain and also reduce the swelling

nonetheless - the best one would be for me right now would be tramadol
which could cause nausea and vomitting
and make me sleepy all day
such a hassle

hence... i just ended up taking paracetemol
ease a little bit of pain
but still.. it's bothering me
.
best option... going to see the dentist asap...
