Treatment of Acne Scarring

The treatments listed below (either alone or in combination) should achieve significant improvement in your acne scarring, but will be dependent on the individual:-

1. Fraxel Laser System

This newly introduced technology stimulates collagen regeneration.  Usually five treatments with the Fraxel laser at four weekly intervals are required for this.  This laser significantly helps to smooth all acne scars.

2. Subcision

Subcision is used to break up fibrous bands that cause rolling scars.  This technique is performed under local anaesthesia by inserting a needle under the skin that is parallel to the skin surface.  It is essential that a dermatologic surgeon performs this procedure as the needle must stay in the plane between the dermis and subcutaneous tissue.  The needle is gently advanced or retracted in a piston-like motion, cutting the tethering bands.  The procedure causes bruising which fades within seven days.  This approach, where appropriate, achieves significant improvement, usually within two to three sessions.  Rolling acne scars respond particularly well to this technique.

3. TCA Cross

With this procedure, high-strength trichloroacetic acid is placed into the ice-pick scar area, resulting in the production of new collagen and re-modelling of existing collagen to fill the scar.  The treatment is localized to the ice-pick scar only and not the unaffected skin.

4. Dermal Filler Injections

Dermal fillers such as Restylane can be injected into acne scars to raise the surface of the skin and give a smoother look.  The injection of such materials does not permanently correct acne scars, so further injections are necessary.   This would need to be repeated regularly every six to twelve months depending on the site of the lesions.  Consideration can also be given to fat transfer into the areas.  Rolling acne scars respond well to dermal fillers.

5. Punch Excision

This can be used for deep scars, such as ice-pick or deep box-car scars.  A punch biopsy is used, which is like a cookie cutter tool that has varying diameters from 1.5 - 4mm.  The size of the punch biopsy is matched to the size of the scar to include the walls of the scar.  Following local anaesthesia, the scar is excised with the punch and the skin is sutured together.  If it is noticeable following this procedure, the area can be treated by various resurfacing techniques.

6. Punch Excision with Skin Graft Replacement

With this approach the scar is excised with the punch biopsy instrument as above.  Instead of suturing the skin edges together, a punch graft is usually taken from behind the ear and placed in the resulting punched out defect.

i know it is impossible for u to understand what daddy is trying to tell u here, u are just 1 year and 9 months old. but maybe when u are old enough and smart enough to read and have ur own fb account, u would find this memoir. hopefully fb would last till that moment.

daddy may not be the best dad in the world. how i wish i could be the one. but parenting is hard Nia. it is tiring and exhausting. thanks to ur mom who handle the job a lot and she did well. I m grateful to have ur mom as my wife. could not ask for more. i wish i could spend more time with u, playing with u , walk u around , watch ur favourite cartoon... timmy , mickey mouse, tayo and the bus , pocoyo... i really want to , but daddy is busy working to pay the bills.

then i know u must be wondering , if daddy is working so hard to earn money , why does mommy has to work too.. well, there are a few reason. firstly because mommy loves being a doctor and being a doctor is a noble job. Daddy is a doctor too of course , but daddy deal with not so sick patients. mostly ladies.... ask ur mommy about the detail occay?

anyway Nia, what i am trying to tell u here is, daddy loves u a lot
u means everything to me
u and mommy are the best thing happened to me. yes , daddy also wishes for a lamborghini and ferraris .. but those are worthless without both of u by my sides

there are times i scolded u.. there are times i acted angrily
not because i am mad at u.. it was because i love u so much and i want u to be a great lady in the future, i want u to be a wonderful daugther
i want to be bring out the best in u. i want u to be 10 times better that i do.

well, this would be the first. i know when u grow up.. u will be scolded more often by me.. it is a part of my job. i have to be the fierce one in the family, the bad guy. mommy is the good and loving mom where u go to tell ur story and love life. maybe when u become a teenager u will hate me and refuse to talk to me when i forbid u to go out at night or i grounded u at home while ur frens are hanging out outside ... and that moment when u r sitting alone in ur room, updating ur status .. " I HATE MY DAD SO MUCH> old fashioned dad! " ... then u will find this entry of mine ... and i hope u would understand my action.. then u will come to my room and kiss me good night... i love u so much hanania shakira...

A ramadhan story - at the bank

I m writing this not to show off or whatsoever
Just something to be shared with the public..
A learning curve for people out there
..

Last week, I went to a bank ( not going to mention which bank, but u r free to guess! )

My atm card was faulty. Frankly I wasn't able to withdraw money from it for about few months already. Hence I decided to get a new one.

I entered the bank at about 11 am

Fisrt I went to the reception

Me : Salam, I want to get a new atm card...  mine is faulty...

She : how can the card is faulty,.. Must be ur fault...

Me : whatever,  just give me the number... 

I get my number and waited to be called by the person in charged of the card renewal. My number was up...

Me : hye, kindly change my atm card,  it is faulty..

He : ok..

And he proceeded with my atm card renewal process...  no problem there

Then I asked him...  " is it possible to increase my credit card limit,? Or change to a new one with higher limit"

" sorry we can't do it. U have to call the credit card center in kl to do so, " he replied...

Owh ok... It is weird.. I got my credit card here easily last time and now u asked me to call kl... Ok... Nevermind...

Then I was asked to see the manager of the bank to validate my atm card...
While she was validating my card...  I asked the Manager again about the credit card....

Me : my other credit card, after a year,  the bank increase the credit limit... But this bank never do it.. My cc limit is the same since 5 years ago....

Manager : owh,  we only increase ur limit if u always pay ur credit card on time...

Me : I never have problem with that..  why don't u check my account....

Manager : Allrite , one second......  Checking.....  owh.... Ooo...
Sir... Would u like to have our premier atm card...  and would u like to have our premier credit card???

Me : owh... Now u offer me??  Nevermind..  I m a bit in a rush today;,, plus the people at Kuwait bank already offered me. They are nice people....

Morale of the sTory : be nice to everyone... 

seborrheic dermatitis - jumpe bomoh

just now i have a mother came into my clinic
bringing her 2 months old baby boy
complaining of rashes all over the body, worse at the scalp
( something like the photo above , just worse )

then i asked her...
" how long ago it started? why didnt u come earlier ..."
any doctor could realized it is already a chronic case..

the mother replied  " about a month ago... the rashes started about a month ago "

"then.. why u didnt bring ur child earlier when it was still in early stage??? " i puzzled ..

" I did.. i brought her to see this traditional medicine people ... diorang dah tiup2 kepala anak... dah baca tu ni.. dah letak daun ni daun tu.... " she answered...


" then? why dont u continue ur treatment with them... might as well... ur baby is full of bacteria n fungal... God knows what the bomoh is doing.. adding more infection.... seriously????  u waited a month? ur baby has been suffering... it is painful.. it is itchy , u cant even spend a few hours in 30 days to bring ur baby to a doctor? this is ridiculous "

I was saddened to see that baby in that very bad condition
there are still mothers out there who believed that bomoh is the first choice of treatment
in this case the baby is in sepsis ... need to be admitted
if she comes earlier, the baby only require topical cream and some oral antibiotic
now there are pus discharge all over body
the baby needs iv antibiotic , need to be warded....

Medical miracles

I was watching the national geographic channel yesterday evening while waiting for breakfasting...
They were showing the medical miracle programme..

In the show,  one of the case is from Malaysia
A 3 years old girl who has cancer and require bone marrow transplant.
The parent brought their baby to Taiwan to undergo the procedure...

I was heartbroken seeing the difficulty and the pain suffered by the parent and the kid herself....

Watching the doctors there doing the job to save the kid reminds me of my hospital days.

I miss the old days. Working at the hospital. Saving life.  dealing with life threatening cases. The adrenaline never stop pumping!

I wish I could continue doing that, being a real doctor...

But it was really tiring. I was mentally exhausted and drained. Have a lot of respect for those doctors who keep working at the hospital. Doing the real job.

Looking myself in the mirror, I am not that doctor anymore. Being a gp and aesthetic practitioner...  I don't deal with life threatening cases anymore. But,  maybe it is the best for me. Aesthetic is my passion, I love beauty. Yes I am a medical doctor and I miss all those heavy duty work in the operating theatre and the icu... But my heart doesn't belong there.

Hopefully other doctors won't follow my step, please work at the hospital,  deal with real patient,  treat real disease.... 
We need u guys. That s why I told my other half,  the best thing u could do for me is to become a specialist doctor.  to compensate for the lost I cause to our health system :)

Why dysport botox is not haram

Seriously I am so tired of people claiming botox is totally haram

Yes the Malaysian fatwa council has decided that botox is haram
But not all kind of botox is haram.
Especially dysport which I am using it widely right now

The fatwa said the botox is haram based on 3 issue
1. Botox is made from pig
2. Botox is dangerous and harmful
3. Botox changes the structure of the face.

Occay,  so let's see..

No 1.... Dysport is not made from pig or its substances; proven by the company. There are many other type of botox which is made from pig.. But not dysport.. So.. We are settled with this issue.

No 2. Botox is dangerous and harmful. According to majlis fatwa... The harmful dosage is 5000 unit and above.
Based on my experience and other Dr...  the amount we used for aesthetic usage of botox is maximum 300unit per patient
, which is about 5 percent of lethal dose. Which made the botox is harmless and not dangerous at all. Nobody in right mind would use 5000unit of botox at one time. Taking 100 pills of panadol could kill u instantly, why is panadol is easily available at any store????

Number 3.... Botox will not change ur face structure. It will only remove the wrinkle around ur face. Nothing more. People will still recognized u; it is just like wearing make up. It tightens the loose part of the injection area. Nothing more.

So there u go..
Please read n make research before u start claiming this is haram... That is haram.. Be smart.  don't make yourselves look stupid ;)
